NAD+: what it is, what the human studies actually measured, and where it stands

NAD+ is the most-asked-about item in this vendor's catalog and the only one that isn't a peptide. Here's the record, in plain English.

What is NAD+?

Nicotinamide adenine dinucleotide. A small molecule in every living cell that shuttles energy and is used by enzymes involved in DNA repair and metabolism. Your body makes it from vitamin B3 and related molecules. Levels in tissue decline with age — that observation is the reason for the interest.

It is not a peptide. Peptides are chains of amino acids. NAD+ is a nucleotide — chemically a cousin of the building blocks of DNA. It appears in peptide catalogs because the same vendors sell it.

The vendor's listing: NAD+ 1000 mg, $200 (30 Aug 2026 catalog). Lot documentation → No NAD+ lot was among the 10 we checked — request one below.

What have researchers looked at?

Infused NAD+ (the form research vendors sell). A September 2026 review found five human studies in total: an 8-person open-label infusion study, an 11-person pharmacokinetic study with saline control, a 10-person safety study of infused NMN, one Parkinson's case report, and a 14-person retrospective comparison. What they measured: NAD+ levels in blood, oxidative-stress and inflammation markers, self-reported sleep. What they did not measure: any health outcome over time.

Oral precursors (NR, NMN) — a different product. Several small placebo-controlled trials show that swallowing a precursor raises blood NAD+ and is tolerated over weeks. They are not evidence about infused NAD+, and they measured blood levels, not health outcomes.

Where it sits on the evidence ladder

Rung 3 — small human studies. Controlled data exist for oral precursors; infused NAD+ has five small studies, mostly without a control group. The review's own words: "Current evidence regarding IV NAD+ therapy remains preliminary and insufficient to support routine clinical or wellness use."

What are the risks and unknowns?

Reported in the small infusion studies: nausea, cramping, flushing and chest discomfort during infusion; no serious events reported in those studies. Long-term safety data: none.

Regulatory status (as of Sep 2026 — re-verified quarterly). Not an approved drug. FDA lists "Nicotinamide Adenine Dinucleotide (NAD)" as a 503A Category 1 substance — "under evaluation" for pharmacy compounding, which is the status that lets licensed clinics offer it while FDA decides. It does not appear on FDA's compounding safety-risk page. That is a materially different position from the peptides on this site, most of which sit in "nominated but withdrawn." WADA: not named on the Prohibited List as far as we found.

Unknown: whether raising blood NAD+ changes anything that matters in a person; whether infused NAD+ even reaches cells intact; what a research-grade vial contains versus a pharmacy preparation.

Documentation checklist for NAD+

CheckWhat "pass" looks like
IdentityThe lab names the molecule (β-NAD, often as the disodium salt) — not just "NAD+"
Purity + methodA number with a method (HPLC) per lot. NAD+ degrades; the reported date matters more than usual
Net contentMilligrams actually in the vial vs the 1000 mg label
EndotoxinPer lot with method, if claimed
Lot IDVial lot matches the COA lot field
Lab lookupThe search code resolves at the lab and the PDF matches

Common questions

Is NAD+ a peptide?
No. It's a coenzyme (a nucleotide). It's in peptide catalogs for commercial reasons, not chemical ones.
Is NAD+ FDA approved?
No. It is under evaluation for pharmacy compounding (503A Category 1). That is not approval.
Is there proof NAD+ infusions work?
No controlled trial has measured a health outcome. Five small human studies measured blood levels and tolerability. Oral precursor trials measured blood levels.
Is NAD+ banned in sport?
It is not named on the WADA Prohibited List as far as we found. Re-checked quarterly.
How do I verify an NAD+ vial?
Same as any lot: vial lot → COA → lab lookup → compare. How to read a COA →
